Have you previously completed training with us and now require a copy of your certificate(s)? Take a look at the scenarios below and you’ll find the steps you need to take to obtain a duplicate copy for your forklift certification records.
Course paid for by you
If you paid for your training yourself, all we will need from you is a written request via email for a copy certificate, containing your name, course dates and truck type to certification@mentortraining.co.uk. Our Certificates Team will then respond with further details including a cost to purchase your duplicate certificate(s).
Course paid for by your employer
If your course was paid for by your employer then, as our customer, they will have received your original certificate when the course was completed. In order to provide you with a duplicate we will therefore require authorisation from them to allow us to release any further copies.
This authorisation should be sent via email to certification@mentortraining.co.uk and must come directly from a member of the HR/Management team within your current/former business. (We recommend that you ask them to copy your email address in on this email so that you can see when authorisation has been granted.)
Once we have received this approval, we will then just require you to provide us with the following information regarding your original training course: your name, company name, course dates and truck type.
Upon receipt of this information, our Certificates Team will then respond with further details and provide a cost to purchase your duplicate certificate(s).
Course paid for by employer no longer in business
If your course was paid for by your employer but they are no longer in business, you will need to provide evidence that they have ceased trading. This can usually be obtained from the Companies House website. Please send this to certification@mentortraining.co.uk.
Once we have received this, we will be able to process your request directly with you. We will then just require you to provide us with the following information regarding your original training course: your name, company name, course dates and truck type.
Once we have these details, our Certificates Team will then respond with further information, including a cost to purchase your duplicate certificate(s).
